The cover of this issue of Clinical Science (volume 136, issue 21) features an electron microscopy image of small extracellular vesicles (sEVs) that play a central role in cell-to-cell communication in normal physiology and disease, including gestational diabetes mellitus (GDM). Using a novel approach involving the continuous intravenous infusion of sEVs with a miniosmotic pump, the study by James-Allan et al. demonstrates that sEVs isolated from women with GDM cause glucose intolerance in pregnant mice because of its impaired ability to stimulate compensatory insulin secretion.
